You can fill out the Coaches Evaluation form:

1. Electronically and email it to the CSC President: Michael Kennedy, or
2. Electronically or by hand and mail it to CSC, P.O. Box 352, Cumberland, ME 04021,or
3. Electronically or by hand and give it to your Team Administrator 

As described on the form, this information will remain confidential with the CSC Board of Directors, and is important in our ongoing evaluation and commitment to our coaches, players and the club in general.
